Section 1: Summary – Overview and Purpose of Position
HS/PRS Case Manager is responsible for fulfilling the JVS mission by providing case management and supportive services to unaccompanied immigrant children. They will meet twice monthly with the Social Work Youth Program Manager to report on cases, and will serve children in Kansas City and the surrounding areas. The purpose of this program is to provide follow up services, case management, and address the needs of unaccompanied immigrant children during their post-release/reunification time.
This position will be housed inside of the Social Work – Youth Program. The Social Work Program is focused on holistic, culturally competent social work interventions, based on strengths, empowerment, and ecological perspectives and adherence to the NASW Social Work Code of Ethics. These culturally competent interventions utilize multicultural and language abilities of staff, as well as trained contracted interpreters. Types of interventions, as well as frequency and intensity of services, are driven by the needs of the community and each client. Building and maintaining positive relationships with clients, communities, and agencies is an integral part of the position.
Section 2: Qualifications – Education and Experience
- Bachelor’s level degree in human services or related field, or equivalent related experience
- At least 1 year relevant experience in case management or similar roles
- Fluency in Spanish required
Section 3: Responsibilities/Duties
- Conduct home visits and phone contacts with the dual purpose of assessing safety and appropriateness of child’s placement with their caregiver (sponsor) and connecting child to services in the community to address risk factors.
- Provide ongoing case management with a focus on screening and making appropriate referrals to address child’s needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, child enrichment activities.
- Provide crisis intervention services directly, as needed, or connect family to services to ensure minor’s safety and well-being in a crisis situation.
- Submit detailed and thorough documentation in compliance with LIRS policies and procedures.
- Utilize best practices with youth for thorough assessments of child’s progress and services provided.
- Maintain regular and timely communication with supervisor around any issues concerning the child’s safety, stability, or well-being in the placement.
- Attend required LIRS meetings and trainings
- Participate in ongoing supervision with JVS Youth Program Manager
- Attend monthly JVS Health and Wellness team meeting
- Work collaboratively with JVS staff to ensure client has access to holistic care
- Work closely with established community partners and identify new opportunities for partnership
- Other duties as assigned.

Section 5: Relationships
Internal: The Case Manager must be able to effectively interact with the program managers and workers within the various departments of JVS.
External: The Case Manager must be able to represent JVS and the Social Work Program to clients, community partners/agencies and funders.
Section 6: Working Conditions and Physical Effort
Frequent travel in the community with private vehicle. Often will require day travel to visit minors but no overnight visits. Setting may vary depending upon the client or partner agency’s location. Moderate physical effort. Occasional may lift/carry up to 25 lbs. May require long periods at a computer.
Salary Range: $35,550 – $43,380 per year
